Method of Gaining Space in Orthodontics | Dentistry.
Gaining space plays an important role in orthodontics. It helps to correct the abnormalities like crowding and malocclusions. here we see about, what are the ways are available to gain the spaces for correcting abnormalities such as crowding, proclination, crossbites, and malocclusions.
there are two types of gaining space in the arch is possible. they are, gaining space by the reduction of tooth material and another one is gaining space without reduction of tooth material.
Procedures with reduction of tooth material
I) Proximal Stripling
II) Extraction
Proximal Stripling
It is also known as Proximal Slicing or Disking or Standardization. The proximal surface of the tooth is reduced to gain space. This method is used when there is less space is required. This procedure involves Investigation, Proximal Reduction, and Fluoride Application.
The investigation involves Carey's Analysis, Bolton's Analysis are carried out. Once Investigation is done, the reduction of proximal surfaces of the tooth is carried out. Finally, fluoride is applied in the proximal areas to reduce the sensitivity of dentin and caries.
Extraction
Extraction of one or more teeth for the purpose of orthodontic treatment is called "Therapeutic Extraction". The premolar is the most common teeth involved in extraction. This procedure is used in the case of crowding, proclination, and malocclusion. Extracting one premolar/Quadrant will provide sufficient space to correct such problems.
Procedures without reducing tooth material
I) Labial Proclination

III) Distalization of Molars
IV) Derotation of Posterior teeth
V) Uprighting of Molars
Labial Proclination
It is done in the patient whose Nasolabial angle is in obtuse angle and it is also done in patients with retroclined incisors.
Expansion
It is carried out in constricted maxillary arch. Expansion is done by various appliances which include jack screws or by use of springs.
Distalization of Molars
It is the procedure of moving molars to the distal side of the arch to gain space for the correction of abnormalities. This method is mostly performed to avoid extraction. Distalization Procedure is done in either of two ways. They are the Extra-oral and Intra-oral methods.
Extra-oral Method
It involves the moving of molars by the use of extra-oral appliances such as Headgear. Headgear consists of facebow which consists of an outer bow and an inner bow. It took support from the cervical region or from the cranial region. Outerbow attached to the neck strap or to the head strap and Innerbow attached to the buccal tubules of the molars.
Intra-oral Method
It is done by the use of intra-oral appliances, which are fixed to the teeth and produce effects continuously for moving the teeth. The Appliances used for the distalization procedures are Sagittal Appliance, Intra-oral magnets, and Pendulum appliances.
Uprighting of Molars
Uprighting of molars provides sufficient space. It is done by use of the molar uprighting springs or by space regainers.
Derotation of Posterior Teeth
The rotated posterior teeth occupy less space compared to the normally positioned molar teeth. The derotation of posteriors brought by fixed appliances with springs or using elastics.
