What is Indirect Pulp Capping?
It is the pulp capping procedure used to prevent the vitality of the pulp. It involves removing caries which is located close to the pulp and placing pulp capping materials such as calcium hydroxide over the affected dentin. And finally, it is restored by restorative materials such as GIC. While removing caries, infected dentin only removed, and the affected dentin is left off.
What is material is used for Indirect Pulp Capping?
The most common material used for pulp capping is Calcium Hydroxide. It forms the reparative dentin or bridge and preserves the underlying pulp. The rate of new dentin formation is 1.4-micron millimetres per day. It keeps on growing till 30 days, afterward, the growth will decrease further. Some authors claim that 390 Micron millimetres are formed at the end of the first year. Earlier Zinc Oxide Eugenol was used as pulp capping material. Recent pulp capping material used is calcium hydroxide-based bio-ceramics.
Indications/ When we can do Indirect Pulp Capping?
It is indicated in the cases where there is minimal pulpal exposure without any symptoms while doing pulp testing. When there is a chance of exposing the pulp during caries removal. In the case of Early reversible pulpitis.
Contraindications/ When we cannot do Indirect Pulp Capping?
It cannot be performed in soft leathery dentin accompanied by exudate or pus while removing caries as it indicates advanced pulpal degeneration. And in the case of grossly decayed tooth. The most important contraindication is pulpal or periapical pathology such as an abscess.
Procedures in Indirect Pulp Capping
The first and foremost thing in any pulp capping procedure is to isolate the involved tooth with a rubber dam. Then excavate the carious lesion with the help of slow speed aerotor. There are Two Types of Doing Indirect Pulp Capping. They are
One Step Indirect Pulp Capping,
It is done in a single visit or appointment. It is done by placing calcium hydroxide and the tooth is restored with the permanent restoration. Experienced clinicians are following this method.
Two-Step Indirect Pulp Capping,
It involves placing temporary restoration in the 1st visit. The patient is recalled after 6 to 8 weeks for checking the formation of reparative dentin. If it is formed, then the temporary restoration is removed and it is restored with permanent restorative materials such as glass ionomers cement.
